Dear Members:
We recently received confirmation for school based SLPs applying for the Speech Language Pathology Certificate at the Arizona Department of Education. You will need to submit a notarized photocopy of your CCC (if they are using the CCC to waive the exam). Do not send the original CCC, you can just make a copy and take it to a notary public to have the copy notarized.
