2. By stopping by the AZ DHS office at 150 North 18th Avenue, 4th Floor, Phoenix, AZ 85007
3. By telephoning the AZ DHS office at 602-364-2536
It is the responsibility of the licensee to submit a complete and timely application for the renewal of the license, along with the required supporting documentation and fees.
