RELICENSURE INFORMATION - It is the responsibility of the licensee to submit a complete and timely application for the renewal of the license, along with the required supporting documentation and fees.
The Division of Assurance and Licensure Services of the Arizona Department of Health Services has a new address and name change. The new name and address is Division of Licensing Services 150 North 18th Avenue 4th Floor Phoenix, AZ 85007 You can obtain your license renewal application form, continuing education approval forms, and other information pertaining to relicensure in any one of the following ways:
1. By downloading them from the AZ DHS website at www.hs.state.az.us/als 2. By stopping by the AZ DHS office at 150 North 18th Avenue, 4th Floor, Phoenix, AZ 85007 3.
